Freedom of Information and Privacy

Privacy Statement

Patient Ombudsman is committed to respecting and safeguarding your personal privacy. This office is legally required to protect your personal information and we meet this obligation by adhering to our internal privacy policies and procedures and the Freedom of Information and Protection of Privacy Act (FIPPA). This office must also comply with the strict rules about the collection, use and disclosure of personal health information set out in the Excellent Care for All Act, 2010.  For more information regarding our privacy policies and procedures, please see the links below:

 

 

Making a Freedom of Information (FOI) Request under FIPPA

FOI requests to the Patient Ombudsman’s office must be made in writing. You may download the Access Request Form to fill in your request. Please clearly identify the records you are seeking and include sufficient details.

A $5.00 application fee is mandatory and must be included with your request. Please pay by cheque or money order made payable to the Minister of Finance and mail your request to:
Box 130, 77 Wellesley St. W., Toronto, ON, M7A 1N3. Under FIPPA, there may be additional charges to process your request that may include the following:

 

  • Search and preparation fees of $7.50 per quarter hour

  • Printing/Copying charge of $0.20 per page

  • Electronic format to disk of $10 per CD/USB

  • Other charges, if incurred, will also be outlined

 

We will respond to FOI requests within 30 calendar days upon receipt of request barring any exceptions such as holds and time extensions.
